The Revised SR21 Form

As the Insurance Institute of Indiana reported in an earlier Insurance News article, the SR21 form, which is utilized to show proof of insurance following a motor vehicle accident, has been revised, as the completed form must now be sent directly to the Bureau of Motor Vehicles (BMV) instead of the Indiana State Police. This change in the law, which took effect on January 1, 2006, should make the entire process of reporting a crash and showing proof of insurance more efficient. The BMV has revised the form to help clarify who the insured is on the form. The title of the form has changed from “Indiana Operator’s Vehicle Crash Report” to “Indiana Operator’s Proof of Insurance/Crash Report” and it is State Form 52441.
